Substance Abuse Counselor

Health Care Resource Centers seeks a compassionate Substance Abuse Counselor to provide person-centered care in home and community settings. You will conduct assessments, develop individualized treatment and relapse-prevention plans, facilitate one-on-one and group counseling, coordinate with medical and social service providers, and support families. Working within a culture of respect, integrity, and professionalism, you will deliver evidence-based interventions, maintain accurate documentation, ensure patient safety, and help clients achieve lasting recovery and improved quality of life.
