Substance Abuse Counselor

Hope Center seeks a Substance Abuse Counselor to support residents with substance use disorders in a skilled nursing and rehab setting. You will conduct assessments, develop personalized treatment and recovery plans, and provide individual and group counseling focused on harm reduction and relapse prevention. Collaborate closely with nursing, rehab, and social work teams to ensure resident-first, dignified care. Monitor progress, maintain accurate documentation, and educate residents and families.
